Should I disclose my health problems on the application form?
It is best to be honest on your job application form so if you are asked specifically about any health problems then be honest. If there is no particular health question then you don’t have to provide this information however these days many employers asked for occupation health checks and or request information about sick leave from previous employers.

Yes, I would – this means they know if they need to make any specific alterations to accommodate you – especially if you are the best qualified person
I think this would very much depend on the job and the health conditions that you have. If your health condition could affect your ability to do the job, then it would be fair to mention it. However, mentioning an irrelevant condition could just cause unnecessary concern and reason for you to be rejected.
However, if the application form specifically asks about health problems, then be honest.
