Have you ever got fired for a big mistake at work?

It is quite rare for people to get fired for big mistakes at work. I have seen one example of this when a temporary staff member made a mistake and was told not to return to work. This seemed unfair to some people as he had been put into a job that was too hard for him and his work was checked by two people who did not notice the mistake. He was young and lacked confidence and this could have had a big impact on his future.

Has anyone ever experienced this sort of thing or been fired themselves?
